Pharmacidynamic test with Isoprotorenol in Dignostic and prognostic role
of the contractile Myocardial reserve in patients with Aorto-coronary by pass
surgery

Material and methods. In this study the patients were divided in two groups.
In the first group there were 26 pts,aged 39-66,mean age 57,9 years,who had
Undergone aorto-coronary bypass surgery after infarctus of the myocard.There
were 19 males and 7 females.
The reference group was made up of healthy persons.They werw hospitalized under
the dignosis of coronary artery diseases,which was not confiemed during the
investigation.
There were 18 persons,aged 59-68,mean age 51,9 years,12 males and 6 females.
Contratractil myocardial reserve (CMR) was studied on the basis of reponse of
global ejection fractio of the lef ventricule (GEFLV),obtained by radionuslide
ventriculography (RNV) to dynamic exercise test( DET) either on the bicycle or
with Isoprotorenol.
Isoprotorenol was given i.v. o,o1 to o,o4 microgram/kg within 5 min.In all pts
GEFLV was made at rest and during DET.
Results.Mean value of GEFLV in 26 pts who had undergone aorto-coronary bypass
surgery afte infarctus of the myocard,was 55,4% at rest. During DET on the
bicycle if fell
For 11,2% and after test with Isoprotrenonl was reduced for 12,8%.Mean value of
GRGLV in the control group was 59% atrest.After DET on the bicicle GEFLV
increased for 5,9%,and Isoprotorenol for 6,2%.
Conclusion: GEFLV is a more sensitive parameter than enddiasolic left
ventriculare pressure.In 26 pts with CHD (after inf.of the myocardium:three
vessel diseases), GEFLV) is reduced.
DET permits the more precise teremination of the GEFLV.Naturally,these
limitations are avoided by the administration of Isoprotorenol.
Aorto-coronary bypass surgery it necessary andergone before infartus (three
vessel diseases) of the myocardium.
